Integrated Health Care

The Centers and Circle Health provide services in locations across Cuyahoga County, offering daytime, evening and same day appointments. Our staff also frequently provides services in community locations and in client homes. Our clients are active participants in their treatment and care coordination plans.

Circle Health and The Centers have come together to have a more positive impact on the Northeast Ohio community. Together, we provide a broad scope of essential services including:

Mental Health Care

Everyone’s brain is different and mental health challenges are common. In fact, one out of five people over the age of 18 have been diagnosed with a mental illness including anxiety, depression, Post Traumatic Stress Disorder and schizophrenia. The Centers offers:

Primary Care

Going to see a doctor is one of the first steps to living a healthy life. Many people have not seen a doctor in years – or ever. The Centers’ primary care program introduces our clients to our team of doctors and nurse practitioners. At an initial appointment: a client’s height, weight, temperature, blood pressure and heartbeat are checked. A doctor or nurse practitioner will talk with the patient about health history and current habits.

Addiction and Substance Abuse Counseling and Care


Drug and alcohol abuse and addiction can happen to anyone. Addiction is on the rise in our community and intervention is key to recovery. The Centers drug and alcohol counselors conduct one-on-one assessments with clients to determine the degree of potential substance abuse and then work with the client, family members or a close friend on a recovery plan which can include: 
⦁ Individual counseling 
⦁ Group therapy

The Centers’ Pharmacy


Along with proper diet and nutrition, sleep and exercise, a wellness plan can include a prescription for medication. Centers clients can receive and pick up a prescription all at one location. If it’s determined that it would be helpful to visit a client at home to help them with medication compliance, The Centers’ pharmacists make home visits.
