What to Bring to Your Appointment

We need PHYSICAL copies of the following for EACH PERSON (including dependents):

  • State Issued ID (License)
  • All Tax Related Forms (W2, 1099, etc.)
  • Social Security Cards (SSN)
  • Direct Deposit (Banking) info
